We have taken every precaution to ensure that your data is protected.

To provide you with the best care possible, we ask that you share the following information using our patient registration form.

• Your surgical referral slip

• A list of medications you are currently taking

• Provide dental and/or medical insurance information

• If you have existing x-rays, you can upload them through our patient registration form, have them forwarded to info@superioroms.com by your dentist or physician, or bring them with you to your initial consultation. If any additional images are necessary, our team will take care of them during your visit.
